HUD Grantee Annual Progress Report Template (PDF, 148 KB)
This report template is being provided to OSA participating states as a tool in discussing the OSA Project with HUD state office staff. This report template is completed by all grantees that received funding under the HUD Supportive Housing Program, Shelter Plus Care, and Section 8 Moderate Rehabilitation for Single Room Occupancy Dwellings (SRO) Program. Certain sections of this report provide information on the number of persons with mental illness who were served by the program, the supportive services provided (i.e. mental health services, case management, etc.) for those who have left the program, and the financial cost of supportive services.
